This Informed Consent document provides important information about Wexford Health Solutions telehealth services. By participating in telehealth consultations and services, you acknowledge that you understand and agree to the terms outlined below.

1. Nature of Telehealth Services

Telehealth uses electronic communication and information technologies to provide healthcare services remotely. Services may include medical consultations, diagnostic evaluations, treatment planning, and follow-up care.

2. Benefits and Limitations

Telehealth can improve access and convenience, but it may not be appropriate for every medical situation and does not replace emergency care or in-person evaluation when medically necessary.

3. Privacy and Confidentiality

Telehealth services are conducted in accordance with applicable privacy laws and our Privacy Policy and Notice of Privacy Practices.

4. Consent to Communication

5. Technical Requirements

7. Emergency Situations

Telehealth services are not for emergencies. If you experience a medical emergency, call 911 or go to the nearest emergency room.
